Register with the surgery

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

New Patient Health Check

You will need to bring a prescription slip from your previous surgery with a list of medication you are currently taking. This medication will be entered into your computer medical record, after being authorised by a GP.

You will be asked questions about your medical history and family history, and may be booked into other clinics if appropriate for your care.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Named (Accountable) GP for patients aged 75 and over

If you are a registered patient aged 75 years or over, our practice will assign you an accountable (named) GP in line with clause 7.9 of the NHS GMS Contract. Your accountable GP takes lead responsibility for ensuring the services we provide under our NHS contract are co-ordinated and delivered to meet your needs, and will work with other health and social care professionals involved in your care to support a joined-up, multi-disciplinary approach.

Please note that you do not need to see your named GP every time you contact the practice — you can still book with any GP or clinician in the team. We will tell you the name and contact details of your accountable GP and explain their role. If your accountable GP is unable to carry out this role for a significant period, we will let you know and, where appropriate, assign a replacement.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access:

  • collect a proxy access form from reception

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
